He is best known for portraying John Gillman / Homelander in Amazon Prime Video's superhero streaming television series The Boys and its spin-offs (2019–present), as well as the dual roles of Jethro and Van West in the series Outrageous Fortune (2005–10) and Lucas Hood in the Cinemax series Banshee (2013–16).

He is best known for portraying Homelander in Amazon Prime Video's satirical superhero series The Boys and its spin-offs (2019–2026), as well as the dual roles of Jethro and Van West in the series Outrageous Fortune (2005–2010) and Lucas Hood in the Cinemax series Banshee (2013–2016).

For his performance in the 2012 Australian film Wish You Were Here, Starr won the AACTA Award for Best Actor in a Supporting Role, among other accolades. He was nominated for the New Zealand Film Award for Best Actor for his performance in After the Waterfall (2010).
